INTRODUCTION:  

The Community Health Representative (CHR) will serve as a liaison between individuals and families in the community and the healthcare center. They will provide basic health care and education to individuals, families, and groups in the home and community to improve health knowledge, attitudes, and practices, and offer support to overcome health barriers. The CHR will provide quality outreach, connection to resources, basic personal care services, and transportation to healthcare services based on individual and family needs.

 

Essential Duties & Responsibilities:

Patient Care (60%)

  • Educate individuals, families, and communities about available health and social service systems. Connect individuals and families to resources and advocate for basic needs (such as food and housing).
  • Home visiting to identify individual and family needs and refer to available services and resources as appropriate including housing, food, transportation, and emergency services. Work closely with Community Health Nurse to identify home health and safety needs.
  • Coordinate services including assisting to schedule specialty appointments and ensuring transportation is available to scheduled appointments and accompanying patients to appointments if they need assistance.
  • Motivate and encourage individuals and families to become more active in their own health care and support them in a healthy lifestyle.
  • Assist with medications including taking medications to homes, setting up medication boxes, reviewing prescription directions to help with understanding how to take medications, requesting medication renewals from pharmacy.
  • Perform basic personal care when other resources are not available (including personal hygiene, dressing, bathing, toileting), housekeeping (including meal preparation, laundry, and housework), and transportation (including transportation for medical appointments and essential shopping) to help with individual health and safety.
  • Support individuals, families, and community while helping to navigate healthcare systems.
  • Work closely with Tribal programs, County Health Department, Adult and Child Protective Services, health care centers, hospital discharge planners, in-home care, and other programs to connect individuals/families/community with available services.

 

Community Health Education (25%)

  • Educate community members on available health programs in the community.
  • Work closely with Community Health Nurse to plan and implement community health education programs.
  • Once trained, take part in teaching health-related programs to help the community, for example fall risk prevention, exercise classes, and safety programs for all ages.
  • Conduct health-related activities and events for community with focus on high-risk community members.
  • Provide community outreach education to local schools.

 

Transportation (15%)

  • Provide transportation to services and healthcare appointments for individuals who have no other transportation resources. Be willing and able to drive in all weather conditions and in both rural and city settings. Work flexible hours to accommodate appointments.
  • Help individuals with their appointments by going in with them and assisting them, if needed, to complete medical forms.
  • Help with care-coordination for any follow-up needed after appointments including finding and accessing resources, and overcoming barriers to care.

 

General Duties:

  • Understand and adhere to HIPAA rights and responsibilities and show professionalism while representing the health center.
  • Communicate effectively and in a professional manner with patients, other staff, supervisors, providers, and outside organizations or agencies as needed.
  • Respond to phone calls, voicemails, mail, and email messages from patients and staff in a prompt and professional manner.
  • Work independently and in partnership with individuals, families, and groups to improve the overall health and wellbeing of the community.
  • Accepts other duties or responsibilities as assigned.

 

MINIMUM REQUIREMENTS:

  • High school diploma or GED. Over the age of 18.
  • Nursing Assistant Certification or willingness to attend certification class within 6 months of employment preferred.
  • Ability to lift to 30 pounds.
  • Basic computer skills.
  • Demonstrated problem-solving ability including planning, organizing, and prioritizing.
  • Ability to identify basic changes in health status of individuals such as weakness, confusion, loss of appetite, and respond appropriately.
  • Basic understanding of Adult Protective Services and Child Protective Services and responsibilities associated with these programs.
  • Required to complete CHR level I, CPR, First Aid, bloodborne pathogen/universal precautions, and defensive driving training within 6 months of hire.

 

 

In accordance with the requirements of the Health Center Manual, employee of this position, as a condition of employment, must document immunity to Rubella & Measles. Influenza vaccine is recommended for all health workers in contact with patients.

 

The employee must be suitable for Tribal employment in your position as determined by fingerprint check results and appropriate background investigation. An incumbent found not suitable after hire will be removed from this position.

 

The employee is required to operate a tribal-owned or leased motor vehicle, commercial motor vehicle, rental motor vehicle or privately owned motor vehicle in the performance of official duties. An appropriate, valid driver’s license is required for the position.

 

 

Tribal Preference Policy: In accordance with the Spokane Tribal Employment Rights Ordinance Tribal Preference will be observed in hiring.
